Enrollment in the Exceptional Family Member Program (EFMP) is mandatory for all active-duty Sponsors who have authorized family members identified with medical (physical, mental, emotional) of a chronic nature (six months or longer) and/or have children who either qualify for or who are receiving special educational services.
Enrollment Criteria
Criteria for enrollment in the EFMP can be found in Section 3 of the DoDI 1315.19 dtd 23 June 2023. Enrollment can be identified:
- During routine healthcare examinations (Military Treatment Facility or Civilian provider
- During the Overseas Suitability Screening
- By self (Sponsor or family member)
Enrollment Forms
- The family member’s medical provider will complete, sign and date the DD Form 2792 Family Member Medical Summary. (DD Form 2792 Instructions)
- The child’s school or Early Intervention Specialist will complete, sign and date the DD Form 2792-1 (DD Form 2792-1 Instructions), Family Member Special Education / Early Intervention Summary if the child is eligible for or receiving services at school. If the child has an IEP or an IFSP, a copy will also be required as part of the enrollment package.

Submission of Enrollment Forms
- The Sponsor and/or family member will contact the nearest Military Treatment Facility (MTF) EFMP Coordinator and provide them with the EFMP enrollment package(s) consisting of the DD Form 2792, and the 2792-1 (as applicable) along with any special educational related documents such as the child’s IEP or IFSP.
- The MTF EFMP Coordinator uploads the completed enrollment package electronically via the Navy Family Accountability Assessment System (NFAAS). Check out MTF EFMP Coordinator for more information.
- The Central Screening Committee (CSC) reviews the enrollment documents and determines eligibility. Sponsors will be notified through NFAAS. The EFMP enrollment letter will state if a Sponsor has been found eligible for the Respite Care Program and/or for Priority Placement regarding base housing.

NOTE: If you are eligible for Permanent Change of Station (PCS) transfer in the next twelve months, begin to update now to avoid unnecessary delays.
NOTE: DoD civilian employees and their family members do not enroll in the EFMP.
